I would think the UTI was a one time thing.  I would guess it was a simple matter of cross contamination.  Which is a general concern, and may mean your Dr's handwashing habits aren't up to snuff....

I don't know about Americans living in China, but circ is pretty much unheard of in Chinese culture.  It was occasionally done by British Drs in hong Kong while under British rule but never routinely.

Possibly he injured it slightly while playing with it.   You don't need to see the Dr to check for a UTI, you just need to send a urine sample to a lab.  I'm not sure how to arrange that with out a regular Dr...

I would put off using bacatracin till I was sure there was a bacterial infection.  It can through off the flora balance and lead to yeast.
