Religious vaccination exemptions...

Do you go to your health department to have it signed? I have someone on another board who is concerned because her Dr. won't sign.

In Kentucky only the parents or guardians have to sign it. You just provide a copy to your doctor when necessary or desired (it is rarely, if ever, NECESSARY to give them a copy). The only ones that really have a need are the schools the child attends, I believe.

Oops, the state was Kentucky. I was asking for someone else because the exemption form provided by the state says this "(Signature of physician, health dept., or their designee) ", but then I check the actual law and it does not require that form, only a written statement.

It is just like the state to try and require more than the law asks for. I think it is an intentional intimidation on their part to make us think we have to comply or that it is just easier to comply.
