Maybe there is a way for you to either find her and the paperwork, or to take care of birth cert yourselves--what state are you in? Was your mw licensed or otherwise 'legal enough' to be a birth certifier?

UCers can get birth certificates, so i don't know why you couldn't.

i know that in my state, you go to the health department and ask for an out of hospital birth certificate packet, and you need a note from a medical professional who saw you pregnant, like a chiropractor or anyone, and a note from a medical professional who saw you and baby after the baby was born. could also be a chiropractor or a pediatrition, or Family practice doc......

you could try calling your local health department and ask about it, just say you had a home birth and now your MW is unreachable to complete your BC paperwork.

It will help a lot if you have anything to certify you were pregnant, especially something official on letterhead. Besides that you and whoever else was present for the birth should be able to sign things. Just go down to the heath dept, they have the form there (first see by phone if you can make an appt to register the birth and what evidence they want on hand). Get it done ASAP, after 7 or 10 days depending on state they make you jump through extra hoops.

There is a sticky in the UC forum on filing for a BC in certain states. I'd look through there and see if your state is listed, or call like another poster suggested.

Yes, just go fill out a UC birthcertificate. But don't wait too long. It gets harder and more complicated the longer you wait.
