Rejoin yarn for an I-Cord?

Ok crafty mamas . I am knitting a doll for my girl and don't know how to rejoin yarn to held stitches for making an i-cord.
So you can better visualize what I am talking about, here is the excerpt from the pattern: I will highlight the important parts for you.

ARMS (Make 2)
Using MC, CO 13 sts. Divide sts between needles and join to begin working in the round, being careful not to twist.
Rounds 1-44: K all sts.
Round 45: K6, kfb, k6. 14 sts.
Round 46: K5, kfb, k2, kfb, k5. 16 sts.
Round 47: Kfb, k4, kfb, place next 4 sts on st holder or waste yarn, kfb, k4, kfb. 16 sts on needles, 4 sts on holder.
Rounds 48-55: K all sts.
Round 56: [K2tog] 8 times. 8 sts.
Round 57: [K2tog] 4 times. 4 sts.
Break yarn and draw through all sts, pull tight. Sew in end securely.
Thumb:
Replace held sts on a double-point needle with RS facing, and rejoin yarn.
Work 3 rounds in I-Cord.
Break yarn and draw through all sts, pull tight. Sew in end securely.

I understand how to make an i-cord, but I don't know what I am suppose to do for rejoining the yarn and which side is RS anyways?

Rejoin yarn just means start again from the ball. Pull out a length of yarn, and start knitting. You'll have a tail that you'll need to weave in later, or you can knit the tail into the i-cord (by holding the working yarn together with the tail until the tail is gone.) I usually leave the tail loose until the end because it's an easy way to see where I started.

Is the doll all in garter st? Does the pattern ever say "rs" or "ws" at the end of the row? Usually odd rows are the right side, but you can just choose the prettier side, probably.

Thanks so much. I sent an email to the creator of the pattern just after posting this and received a reply back within 10 minutes. I didn't expect such a quick response. I also didn't think to go right to the source until after I posted this. Between both of your responses, I am pretty sure I can finish the thumb on this doll's arm.

How do I keep from a gap from forming between the thumb and rest of hand? There is a small hole where I did exactly as told.

For anyone who is interested, this is how I am doing the second arm, and I might frog the other arm at the hand and rework it this way too.

'For arm, instead of moving 4 thumb sts to holder, I worked the i-cord thumb right away, sniped the yarn long enough to finish hand with and pulled it through all sts and down i-cord before continuing with hand instructions.'

I am excited to try it this way and in the future for other parts that require sts. be put on holders.
