Physician's report for school

I am enrolling DS in a private nursery school in January. He is unvaccinated. The school asked that a "physician's report" be filled out by DS's doctor as part of his enrollment paperwork. I just picked up the completed form from the doc's office and see that he left the immunization section totally blank. Do I submit it "as is" and wait for the school to question the form, or do I need to write something in that section to indicate DS's vax status. If so, what should be written?

I would leave it blank and if they question you, hand them a religious exemption...but that's just me

Our pediatrician leaves the immunizaiton section blank, too. What I did is write a letter to the school, stating that we are exempt from immunizations on religious grounds. I gave our medical history form with the letter to the director. I told her we were exempt on religious grounds and asked her if hte letter was sufficient or if she had a certain form for us to complete. Note. I'm not asking if its okay. My attitude is "this is no problem. How do you want to handle the paperwork." Personally, I wouldnt' just submit it without saying something or attaching a note that you are exempt on religious grounds. Why raise anxiety and concerns about why the section is blank? I'd try to minimize any anxiety, discussion, etc. about the topic. Why put immunizations on the horizon any more than you have to. Priv. schools can exclude you, I believe, for failure to immunize.

Leave it blank, the school may provide you with an exemption form.

Depends on the state. Exemptions have to be accepted for both private and public schools in some states.

Thanks for the replies. The school has its own waiver form, which I signed, and that was it! No questions asked, and no hassle at all. What a relief!
