Past "due" and need advice

Let me start by saying I am really grateful for my midwife and very excited to have a HB. That being said, I swapped care providers at 30 weeks in order to have a HB. The only HB MW covered by my insurance lived 2 hours away, but agreed to take me as a client (I think because she felt how strongly I wanted a HB). I made three trips (2 hours one way) every 2 weeks until 36 weeks for prenatal appointments. Her web site states that they do home visits every week after 36 weeks. She came to my home at 38 weeks with her assistant / doula. At that meeting, they mentioned doing a fear release exercise next time I came up (her assistant is a hypnotherapist). I paused and said, "Next time I come up"? We decided that if everything was going fine that we would just speak on the phone. I didn't hear from her again until I was 41 weeks and she said that if I go to 42 weeks, she would not feel comfortable not having seen me in awhile...that she feels like she should be monitoring my B/P and the baby's HR at this point.

Of course I agree, but she didn't suggest coming down here. By now, she would have made 4 home visits to a client that lived closer and I'm assuming it's because I live so far that she is not willing to come until the birth. Usually they do PP checks 1 days, 3 days, 2 weeks, and 6 weeks PP too, but it sounds like she is only planning on coming for the 6 weeks if I need it. She is planning on staying for 4 hours after the baby is born to make sure we are doing OK.

I am now 41 weeks and 3 days pregnant. This is my third child. I am low to no risk. I checked my B/P yesterday and it was 125/64 and my HR was 71. The baby is active. I am nervous though as the days wear on and am considering calling my prior OB to go in for an NST. I was induced with both prior pregnancies - my first for being "overdue" and my second for PROM. I am not planning an induction this time around and my MW is sending some herbs (blue and black cohosh) to start this week.

Would you go to the hospital 3 minutes down the road just to "check things out" with the baby? I am not driving a car for 4 hours so that she can check the baby's HR and feel my belly and I don't feel comfortable bringing up my concerns because she is willing to come and help birth this baby. Please help!

My midwife was able to order me NST's at a local radiology office; is that a possibility? Yes, I would definitely want to get things checked out. And, gently, I think it is a little unreasonable to expect her to drive 4 hours to come check on you at your home... although she definitely should have discussed that with you when she agreed to take you on as a client!

What I would do: call and request NST order with local radiology clinic or hospital; let her know your concerns; ask her specifically what her plan is to continue monitoring your pregnancy. I went to 42 weeks and after 41 I was getting NSTs every couple of days.

I too, think the drive is unreasonable for just checking in, but after not seeing her for almost 4 weeks, I'm not sure what's more unreasonable - me driving for 4 hours at almost 42 weeks pregnant or her. And I'm somewhat upset that I didn't hear from her for three weeks after the home visit.

As for ordering a test, I'm not sure that is possible as she is a CPM, not a CNM and I don't know if she can order anything or analyze it? I asked about her plans for if I go past 42 weeks, and it seems like she would 1) want to see me and 2) is planning on the black and blue cohosh putting me into labor this week.

This is why I am torn, thinking that I should go ahead and call my Drs office because I could have the NST there and have it looked at.

I had a long distance MW...We alternated weeks who drove rather than her visiting me..She was about 1.5 hours away from me. She was a CPM and was able to order and send me for a NST and BPP (I'd want both at 41 w and again at 41w3-4d and then at 42w and every 2 days thereafter, but I'm pretty terrified of negative outcomes). I went to her at 41w so we could do the testing, she'd come to me the week before. They didn't do a 24 hour PP check up but did a 4 day PP check and I went to them at 2 weeks and 6 weeks.

I wonder if there would be somewhere halfway you could meet if you needed to.

I think the issue here is what you feel comfortable with at this point. What do you feel that you need, and have you discussed that with your midwife?

personally, I would go see "someone local" for an NST and/or biophysical profile. I wouldn't go to the hospital for that, just because they could terrorize you into staying for an induction.

I'm sorry that she doesn't feel like it's justified for HER to drive to YOU, but I hope this is the only hiccup in your care.

I'm not sure that she doesn't want to drive to me - she just hasn't suggested anything about being past due other than that she expected me to go to 42 weeks (I did with my first, as many do). I think she cares, she just feels that there genuinely isn't any concern and that everything is fine. I suppose it is up to me to make suggestions?

I think I will try the blue and black cohosh, contiinue to feel the baby moving, and if I still don't go into labor by 42 weeks, then I guess I will ask her if she can't order tests down near me. I would really prefer not to make that drive. I forgot to mention that I have two other children under the age of 3 so every long-distance trip requires planning in terms of care or what-not.
