Exemption for the physcial?

Where do I find a spot for exemptions on the physical? I don't really object to a physical, but I do suspect it would get dicey with the pediatrician asking him/her NOT to fill out the vaccination section... Anyway, I found the religious exemption spot for immunization, and for lead blood testing, but not for the physical.

I don't think there is one. The forms they handed me the other day at kindy registration had some blurb on it about exemptions that said the physical could also be exempted, so I high-lighted that, and attached those forms to the official vaccination form that has the religious exemption. So far no problems, so hopefully it will stay that way.

For preschool, I just wrote a note on teh physical form about using the religious exemption.
