Ds is going to be starting preschool in the next week or so. Part of the paperwork is an authorization for them to give emergency treatment. Of course I am ok with that. However I'm wondering if there's some sort of caveat I can add to it so they do NOT try to give a vax? I know vaxes aren't an emergency but on the sheet it specifically asks for the date of the last tetanus shot.
The main part of the authorization says:
Then it asks for names of parents, doctors, emergency contact, and last tetanus shot.
Do I need to worry about this? Can I add something to it?
The main part of the authorization says:
Quote:
| I (name of parent) hereby authorize any physician or member of the Dept of Emergency Medicine....and/or any member of the Medical Staffs to render medical treatment, which in his/her judgment may be deemed necessary in the care of (name of child). |
