Edema in the nipples, areola...any way to ease this?

OK this is a new one for me!

This is my fourth babe, and got a plugged duct over the weekend. Well, really bordering on mastitis...fever, chills, red area on breast etc. I got to bed, nursed as much as possible, did heat, breast compression, pumping etc. Finally got the hard lump to disappear. In 24 hours the fever broke etc. and I was feeling better. Took it easy for a few days, yet continued to express, etc. as the lumpy area got engorged and hard.

Fast forward a few days. Apparently the pumping has caused some injury to my areola/nipple. My nipple is quite large and would *just* fit into the flange on the pump. Well, now, the nipple is quite firm, swelled, as is the areola. And, I use the term edema because really it's acting like that...when I do get him to nurse on that side it's quite painful, the nipple isn't stretchy like usual, and after he gets in and nurses, the nursing is very slow (slow milk flow in comparison to the other side). When he unlatches I can clearly see the indentation in the areola that he has made from the tissue that was in his mouth. And, he is getting enough areola into his mouth. No color change, just a pitted area. I also notice that if he puts his fingers by the areola, it will leave a pitt. It's really acting like pitting edema. I also get a deep painful ache after he is done nursing as the fluid returns to the area. The areola area is very full/stiff as is the nipple. Hand expressing is quite difficult, if not impossible. Compression in the area is quite painful, as is nursing initially as the nipple gets stretched at the beginning of a feeding. However, the actual breast feels completely normal, like the other breast. It appears to only be the areas that came in contact with the pump that are experiencing the edema.

I'm not concerned about supply, honestly I generally have oversupply, and literally I'll nurse him all day if I need to...he's having generous amounts of poop/wet diapers and is doing well.

I generally don't pump, have never used a baby bottle either. Likely got a bit overly assertive with expressing when i had the clogged duct (bordering on mastitis) as I didn't want to have to deal with antibiotics, etc.

I'm concerned about comfort levels, and if anyone has any suggestions on things I might try. It seems that only the areas that came in contact with the pump are affected. And, it IS swelling, not engorgement. Having functioned as a breastfeeding counselor in the past I know the difference. This just doesn't exactly fit with the typical 'sore nipples' I used to council on and I'm wondering if anyone here may have some different suggestions to try. I have read through my resources, the breastfeeding answer book, etc. and haven't really found anything that directly addresses this type of situation.

Only have one minute to write, but I had the same experience recently that also came from the pump, I think. Mine happened in conjunction with mastitis.

I found poke root (which is a lymph cleanser) to be extremely helpful. I took the extract, and used the oil topically.

Well, I don't think I could get a hold of that herb in time really. Though it's reassuring that someone else has experienced the swelling in the nipple/areola from pump irritation. It seems clear to me now that I should NOT pump again on that side ever...and if I did I'd have to get a special large size flange!

The issue seems to be swelling, with no fever or any evidence of mastitis in days I believe all the damage I've done is from the pump, mainly as it's only where the pump came in contact with the areola/nipple.

My thoughts so far:

Ibuprophen every 4-6 hours per day

Cold pack (don't know if ice is good, though I have some pads that are cold)

Increase the amount of time between nursing sessions on that breast. I was doing that breast at ever feeding to get rid of the mastitis...now that it's gone I probably should go back to one breast per feeding/3-4 hour period.

All of this is aimed at reducing the inflammation/irritation. Hopefully this and a few days will take care of it.

If anyone else has any suggestions I'd appreciate it!

I would do what you are doing now and keep an eye on it. If it doesn't go down in the next day or so, you could still have an infection or an allergy somehow, that got into the nipple.

No signs of thrush? What about using an ice pack on the nipple? (I know it's not comfortable sounding). Heat packs can actually make infections worse by helping the bacteria grow faster.

Just a few things off the top of my head!

Well, if it's thrush it would be unlike any kind of thrush EVER. LOL.

Actually so far it seems to be the same. I'm pretty perplexed. Called a couple of friends-LLL leaders and they're perplexed too. I've never seen anything like this either.

I'm almost thinking of just trying a couple of different things...like garlic orally. Just chopping off a few cloves and swallowing that away. Then more probiotics.

I was hoping ti was something that time would help improve...a few days it's not really any different. Sigh.

Very little change if any....still has the edema, etc.

I have started garlic, about 4-5 cloves a day. Hoping something is going to work.

This is very frustrating as this is usually my favorite side and my babe's favorite side. Sigh.

Thanks so much for that link. Interestingly enough I discovered all of that on my own, didn't know there was a technique. Though my damage and edema was caused by excessive pumping with a pump of an inappropriate flange side STILL definitely it has been helpful!

As of right now on the right side of the nipple it's improved about 80% and on the left side about 30%. I'm hopeful that within another week it will be completely back to normal! However, my milk flow is excellent on that side, and thankfully I'm starting to feel like this is temporary.

Reading your link I have to think though, if someone went through this as a result of a hospital birth, etc. how frustrating it would be. I'm happy I've not had any major issues ever with breastfeeding, and have been able to have good births at home (milk comes in within 2 days).

Knock on wood...my edema appears to be gone. No more pain! Woo Hoo!

Only took about two weeks! At least now I can call back my LLL friends and tell them how things went. It really was just time that healed it. Boy am I happy now as that was my favorite side to nurse on!

Well, this is very late, but isn't the cabbage in your bra trick considered to help swelling as well as engorgement? I know the engorgement I had with dd1 (terrible, terrible) was mostly tissue swelling and less milk engorgement. I did cold cabbage leaves for 15 min at a time to good effect.
