Proof of Pregnancy for Insurance

I am 12 weeks pregnant and trying to get Medical Assistance. I did to have my provider sign a Proof of Pregnancy form that I need to send in so I can get the insurance. The thing is, I am going UC, so I don't have a provider. Is there anywhere I can go that provides free pregnancy tests and would maybe be able to sign this thing? (And please don't say Planned Parenthood.)

If your provider signed a proof of pregnancy form, then that's all they request. Is medical assistance requesting more from you for proof?

Unless you mistyped and are saying you "have to have your provider" and have not done it. If you have a primary DR you feel comfortable with to just go to for the test and to sign off on, that's my first suggestion. MA will pay medical bills up to 3 months prior to your approval if you submit the bill to them. Otherwise, ask at a WIC office perhaps? Some have OB type services (at least the one I was at in WI, not sure how many actually do), but they could maybe point you in the right direction.

Our local health depts will do pregnancy testing once a month....you could try there. Otherwise I would suggest going to your Primary care Dr & asking them to do it. Just tell them that you need this done in order to get the Ins ~ no need to tell them your plans to UC/UP
