example of an exemption letter for Ohio schools?

Does anyone have an example of an exemption letter for Ohio schools to use when registering my daughter for kindergarten?

Scroll down past the newborn stuff to find examples. If you object to anything on a school district provided form I don't believe you have to sign it - you can submit an affidavit (they have examples of those too). If the form isn't objectionable then I would sign their form.
