Any Texan Moms who can help?

I vaccinated my older children but I am not vaccinating my younger kids. My oldest daughter in high school came home with a list of vaccines they want her to update. I would like to sign an exempt form for her. I have tried to look online and it makes my head spin. Anyone in Texas that can help me navigate to find the right forms?

Doesn't the school have an exemption form to fill out? I believe that Texas has the philisophical exemption, so it should be very easy.
