We have the same problem. Often errands have to be taken care of and I have no choice but taking dd (2) with me. And she is a runner, and hates to be in a stroller or cart. We have the monkey back pack though I find that its not very effective once dd has decided to be uncooperative, then I either have to carry her or drag her back to the car which is difficult and exhausting. I have found what works best is to give her a choice before we get out of the car. I say do you want to ride on my back (in the mei tai) or in the stroller (or cart if we're at costco, grocery store or target. She usually says "I want to hold your hand" So then I say "okay, but what happens if you run away" and she says "you put me back in the stroller (or cart). And yes she does usually throw a fit if I put her back in the cart or stroller, but at least she's safe. And that's usually my cue to finish up as quickly as I can, but I never just walk out, we always finish what we came to do and if she screams, she screams. The other thing I do is try to make the visit as fun for her as possible so that she would rather cooperate than have to leave. When we go to the mall we usually stop by the play area when we're done and when we go to target we always stop by the tricycles and take them for a ride. Also sometimes I let her hold a toy (though I emphasize that it's just for looking at we aren't buying it which she is totally fine with) while we walk around the store. Also bringing a snack that she can sit and eat in the cart or buying a treat to eat while sitting in the stroller really helps a lot as well.
