DS says "ouch ouch ouch" last 2 times he peed

In addition, the tip of his penis is red and apparently sore. He said "ouch" when I put ointment on him.
His foreskin doesn't yet retract even a little.

I was thinking UTI, but he doesn't have a fever and plus he is sore when not peeing.
I am giving him cranberry extract just in case (can't hurt), but I'd rather keep an eye on it than rush him in and have docs trying to cath hm and/or retract his foreskin.

I am thinking foreskin seperation. This can happen to boys usually before 5 or so it's a growing pain that a few might experience. As the foreskin separates it can leave some raw spots where it was once attached which might sting when he urinates. It should pass. We have a thread that talks about it which I'll bump up for you.

In addition to painful urination and occasionally a fever, a UTI would proabably also have frequent urination, cramping in the pelvic area or back and the urine often has a strong smell, looks cloudy, or contains blood. This is a sign of pyuria, or a high white blood cell count in the urine, and is a very reliable indicator of urinary tract infections.

I'll bump the thread for you and I am betting separation.

Thank you so much! I'll go and read that thread. He has none of the classic UTI symptoms.

Also consider if he has any swelling or discharge it might be yeast. When my son was two he spent a lot of time in wet swim diapers and developed a yeast infection. He complained that it hurt when he would pee, he had a lot of swelling that happened over 24 hours (we first thought it might be separation but the swelling caused concern) and then he developed a discharge.

We bought some OTC yeast cream (Monistat or something like that) and it took about 10 days of smearing it all over his penis and leaving a glob of it on the tip of his foreskin at each diaper change to clear it up.

Yes, could be just normal separation especially if you can rule everything else out. DS had this happen 2-3 times now, and it never lasted more than 2-3 days.

What you can do is try to get him to pee in water, like in the tub or in a cup. Make sure the whole penis is submerged in the water. This should help dilute it and take the sting away. I tried with mine but since I never had him pee in the water before, on purpose, he wouldn't do it.

No swelling at this point, but I'll keep an eye out for discharge.
He's no longer red, but he's still freaking out a little when he pees (not like yesterday, though).
Hopefully he is on the upswing.

Does it matter how much? I'll try a cup or so.

I keep taking his temp and no fever, but he cried tears last time he peed.
Otherwise, he runs around like everything is just fine.
I feel really helpless and I hope this quits soon.

I never measure but you probably don't need a whole cup though. Baking soda baths really helped last year when my ds1 was having soreness , swelling and stinging when he peed. It cleared up in a couple days after I started giving him baking soda baths. I also gave him at least one vinegar bath too (Ijust pour some in but you want to make sure you dilute it enough).
