Red painful tip in 7 month old

This morning when we went to change my ds (7m months) the tip of his penis looked VERY red and sore and possibly has some discharge. It's been looking worse all day and he screamed during his last diaper change when I cleaned it so I'm sure it's sore. We normally use cloth diapers and cloth wipes but we are on vacation so we are using disposables. Could it be a reaction to this? He also just finished up a round of antibiotics last week for a double ear infection...any way it could be a reaction to that? He is also teething and I wondered if that could be a cause?? I'm concerned that he's in so much pain. He has been fussy and feverish but we figured it was due to teething but not really sure.
Oh, he is intact and we have never pulled it back at all.

Ok my first thought when you said abx is that he has yeast from them very common thing to have happen. Does he have a rash on his bottom or scrotum or is it just the foreskin?

Get some 7 day OTC monistate and slather that on there really well if it is yeast you will see improvment pretty fast. If it isnt then it is probably a bacterial infection. That can be treated with success using OTC bacatracin in a lot of cases. Try yeast first then bacterial meds. That is what I would do.

Since you're on vacation, two strong possibilities are that he is reacting to the chemicals in the sposies and/or wipe, or that he is spending too much time in his carseat and the strap that goes up between his legs is rubbing. If he has been left with a babysitter, another possibilty is that they retracted during a diaper change.

As well as what MCat suggested incase it's a yeast infection, switch brands of sposies and wipes, don't drive anywhere if at all possible, ask anyone that has watch him how they cleaned during diaper changes, and use a bit of diapercream on it.

ETA: if you're at a place with a pool it could be the chlorine.
