Rules about (and where to find) vax forms

Does anyone have suggestions on where I can find a vax exemption form for my state? And is it going to be an issue if I fill one out even though he's had a vaccine? This is for day care, so they definitely need *something* on file or else he can't go!

I live in TN and was able to just get on the state's website and I typed in immunization exemption forms and it pulled it right up. I did just find this website that lists different states so you could check and see if your state is listed. http://www.unhinderedliving.com/statevaccexemp.html
