What to put in a Letter from Dr.?

We are asked to provide a letter from our doctor stating our refusal to vaccinate due to allergies. So our ND asked us to draft a letter and then she would look it over and sign.

Does anyone have a template or is really good with wording?

Who is this letter for?? If you are trying to use it for school, NO state (that Im aware of) accepts a medical exemption from an ND. It must be an MD.
