Where do I get an exemption form?

We're in the state of MN and I need to file and exemption for my dd who may be entering public school next year and my son who will be entering a special needs class. My son can easily get a medical exemption but my dd will not be able to. I believe MN offers other exemptions though.

How do I go about getting one and do I need to have it notarized or anything like that? TIA!

It's on the same form that you have the doctor fill out if you are vaccinating - you just have to sign it and get it notarized.
