Getting desperate

A bit on my background first:
My first baby was born with pitocin/epidural. I received no breastfeeding advice or support. He was not gaining enough, we supplemented with formula, my milk dried up in a few days.
My second baby was born with pitocin, otherwise natural labor. I read a lot about breastfeeding, we had no problems at all and she breastfed for 3 and a half years.
My 3rd baby was born completely naturally, nursed 5 mns after birth. However her latch wasn't great even though her sucking is strong.
At 3 weeks 3 days she is still not back to her birth weight of 8lbs4oz. This morning she was 7lbs13oz. She has gained only 2 oz in 10 days.
On tuesday I saw my midwife who after touching my breasts found that I have a low supply. I've been taking fenugreek and I have a lot more milk now.

I feel that there are several issues:
1- Her latch in spite of seeing my midwife and 2 LC is still not very good. She keeps pulling her lower lip too close to the nipple even when she latches on fine. I keep pulling it down, she keeps pulling it up. It doesn't hurt when she is at the breast though after a few seconds.

2-She loves to sleep at the breast, using it as a pacifier. She does that too much instead of eating.

3.I've never had too much milk even with my second and I'm not successful at pumping..

She is fed as often and as long as she wants to. How is it possible that she is not gaining weight?? I'm also doing the breast compressions.
She has been seen by the midwife and 2 docs who all think she is very healthy. She certainly seems to be: she holds her head great and is strong. She has been smiling at us since 2 weeks old and is very curious of her surroundings. She has a strong hearty cry. She has at least 4 dirty diapers a day, most days more like 6 plus 2 wet diapers.

For the first time today the doc mentioned supplementing. I'm afraid that if I do that my milk will dry up again but I'm very distressed over this and I've been crying every day. My midwife still thinks that just fixing my milk supply will do it and that if i EBF once then I can do it again. Her frenulum is quite close to the tip of her tongue and the doctor when I asked her checked her mouth thoroughly for signs of tongue tie but found that her tongue goes easily past her gum and so it shouldn't be a problem.

What would you do?

It sounds like she may not be transfering milk well.

Have you checked out the tongue-tie stickie at the top of this forum? Extending past the gums is just part of it - the tongue needs to be able to reach the top of the mouth too. Do you have access to an LC who knows about tongue ties?

Also, remember that there are medications to help boost milk supply as well as herbs and foods. Moms in Canada are sometimes temporarily prescribed domperidone to bump up supply.

We were supposed to bf, do compressions, and switch sides as necessary to keep her bfing. No more than 20 mins though (Olivia would never go that long, she could now though, yay!) and then I had to pump and feed her whatever I had just pumped. I have a Medela Symphony I'm using. She would stay awake and nurse well as long as the milk flowed easily, when it got harder she just went to sleep. At a week old she was 7#13oz, 4 days later she was the same and that's when we saw the bfing DR and started on our routine. 3 days later she was 8#3oz, 1oz over her birthweight and today she was 8#9oz, so 6ozs in one week. We're not pumping as much as we were and haven't needed formula since Friday and then I almost pumped the amount of formula we used after she went to bed. I also started Milk Thistle and Saw Palmetto Berries for supplements and have seen an increase, more than anything else I'm taken. I've also taken fenugreek and alfalfa since she's been born. I saw instant results w/ the new supplements though.

You said you're not sore etc, so tongue tie probably isn't an issue but it can't be dismissed IMO. Olivia has a slight tongue tie, but since she's doing better now and I'm not sure I've decided it's not a problem. The dr wanted us to try the things above first, the pump and feeding before considering the tongue tie since hers is so small. We were going to to revisit it in a few wks though if things weren't improving. Olivia also has problems getting enough of the bottom part of my areola in her mouth, we're working on it and it'll get easier as she gets older. Your breast is going to be harder to empty though and won't get the right stimulation from her w/out hitting the breast right though.

My goals were:
#1 feed the baby, didn't really matter if it was bm or formula, baby needed to eat. A hungry baby is a sleepy baby often, we found that out really late w/ my last dd.
#2 increase supply
#3 get baby to breast. This is our ultimate goal, but for short term we were more concerned about the other 2 first.
