Information about Patient 1 in various outbreaks?

Anyone have links showing whether typical outbreaks of vaccine-preventable diseases are started by unvaccinated children or vaccinated children?

How do you go about finding "Patient 1"? Didn't patient 1 have to contract the illness from someone as well?

It's easy to find information pointing the finger at unvaccinated persons or those with unknown vaccination history, like the outbreak in San Diego. Does this keep coming up because the incidence really is so much more prevalent in unvaccinated persons, or because the CDC wants to make an example out of these cases?

There is this case: (Patient 1 is referred to as 'Student A' or 'index patient')

This one doesn't mention the index patient but shows measles outbreaks occurring in immunized patients and calls for another dose of MMR to be added to the schedule as a result...

Not necessarily, live vaccines can, very rarely, create a patient 1 out of nowhere.

It is going to depend on the VPD.

Measles is usually brought by importation and spread via and through unvaccinated populations. We can see this in San Diego, Arizona, and PA outbreaks--90% of cases last year were imported after travel to active outbreak areas.

Whooping cough, on the other hand, can go either way. People who see unvaccinated kids as a big source often cite this study:

Aw yes! doh!

In fact do you really think it's that rare, I thought shedding was quite common? Varicella... MMR, are more frequent, no?

More rare just because there not even adminstered as much, are reports of transmission with OPV, and I think I heard of a case where a father in the military passed on small pox to his 2 year old...

that would be interesting to find research on too...
