Mostly true, however some countries (mostly African contries I believe) do require the Yellow Fever vax or proof of immunity. Most other countries just require the vax if the traveller is coming from a country where there is a risk of transmission.

I don't remember as I haven't flown into HK in years, but they might well require proof of yellow fever from a traveler from a "yellow fever" country. Could even be on the immigration form you have to fill out on the plane. However, yellow fever is moot point for a traveler from the US.
