HB with physiological 3rd stage or actively managed

I have physiological down for my bp right now but was curious as to what is more common with Homebirths, and wanted opinions either way...as I dont really have an opinion either way. I have NO IDEA what happen with my first in the hospital, as I wasnt asked anyways.
Opinions? Negs/Pos?

Would you mind explaining what you're talking about? Are you asking about routine interventions, herbs, etc. to help expel the placenta?

I believe the WHO recommends active management for hospital births only.

Yes I mean the ergometrine-oxytocin shot given as the baby's shoulders pass through the birth canal. I have a choice on this (Well, as in to give as described, or to wait for the placenta to naturally expel or recieve the shot)
What do they do in the US? I dont remember getting a shot.

I've never heard of giving a shot as a routine intervention, even in hospitals. I wouldn't get it unless there were some specific reason to. I can't imagine a HB MW doing that routinely.

Thats what I was thinking but my MW made it seem as if I was "opting out" by not taking the injection. It seems its routine here (scotland) in L&D and the midwife unit.

I think in many (most?) US hospitals the 3rd stage is actively managed with pitocin and traction on the cord to remove the placenta.

I do not think that active management of 3rd stage is at all common for HB midwives in the US; in fact, I would think that active management violates the midwifery model of care in general. IME, most HB midwives carry herbs or pit in case of PPH and would only administer as needed.

I had PPH with my last HB and consented to a shot of pitocin in my thigh to help manage it. Everything was fine.

here most HB midwives don't "actively manage" third stage. but some do have time requirements. like if the placenta isn't delivery in such in such amount of time they will start with Pit, traction or herbs etc...
every midwife is different.

personally I wouldn't want any "preventative" management unless there was reason for actual concern.

My first birth was in a hospital and even then the OB didn't do anything but wait for the placenta to come out on its own. It didn't take long, so I don't know what she would have done if it hadn't.

Needless to say the midwife didn't do anything at my home birth either. (Actually my birth was so fast she missed the birth and the placenta was coming when she arrived.)
