Um... WHO do I submit my DD's exemption to?

I'm in MA if it matters.

"No child whose parent or guardian states in writing that vaccination or immunization conflicts with his sincere religious beliefs shall be required to present said physician's certificate in order to be admitted to school."

I've searched this site and read about 10 threads about exemptions in MA. I've also read other sites like http://www.vaclib.org/exempt/massachusetts.htm

I seem to be missing something that everybody else seems to understand - where do I submit this to?

Do I just send it to the school or whatever? I guess I was thinking you officially filed with the state or something; am I wrong?

Exemptions are only for school (and sometimes daycare). You do not need one for a doctor, medicaid, WIC or anything else. Just school.

Thanks guys. Honest, I have a college degree and everything. I guess I just made an assumption and couldn't find information direct enough to challenge that assumption.
