WA state exemption

I'm filling out the forms for public school...

Is it just as simple as checking the box for exemption on the form or do I need to compose a letter as well. Do both need to be notarized?

It really is that simple. You don't need to have it notarized.

To clarify.... No notarization is needed. But no letter either? Just exemption form is all I need, correct?

I've submitted two exemptions, one to a private preschool and one to a public school. No questions, no additional info required. It was so easy. Legally, the form is all that is required. I guess it's possible you may be asked for a letter or something but I certainly wouldn't offer it.
