If you are in Ohio... (is this letter good enough)?

What did your exemption letter say?

Is this good enough, or do I need to say more about our religious beliefs? I want to keep it as simple as possible.

We, _____ and ________, as the parents of ________ are exercising our rights, due to our sincerely beheld religious beliefs, under sections 3313.671 and 3301.07 of Ohio revised code to receive Religious Exemption from Vaccination, ALL injections, prophylaxis, & testing due to our genuine and sincere religious beliefs which are contrary to the practices herein required.

The Ohio Revised Code 3313.671 Proof of required immunizations - exceptions states that:
(4) A pupil who presents a written statement of the pupil’s parent or guardian in which the parent or guardian declines to have the pupil immunized for reasons of conscience, including religious convictions, is not required to be immunized.

(5) A child whose physician certifies in writing that such immunization against any disease is medically contraindicated is not required to be immunized against that disease."

I would leave out # 5 since it does not pertain to you, right?

And I would add something like


Since the practice of vaccination and the injection or application of any foreign substance is contrary to (my/our) conscientiously held religious beliefs and practices, and violates the free exercise of (my/our) religious principles, we are submitting this vaccine exemption letter.

Would you please be so kind and place it in our (daughter's) school file.

Just a thought, but I wrote my letter, then had a lawyer who is knowledgable in this, area proof read it for me. Gave me peace of mind and someone to turn to incase I was given any problems aftre submitting the letter.
