Illinois vax exemption rights

i wanted to put my son in the federally funded two rivers head start program. when i explained that i already had my own exemption form they said they have one they need for me to fill out.

Parent or Guardian Waiver
(Health Examinations, Immunizations, Vision & Hearing Screens/ Exams, & Dental Exams)

Iam exercising my right, under the State of Illinois Administrative Code Title 77 Chapter 1 Subchapter I part 665 Section 665.510, to object the following state requirements based on religious grounds:

Health Examinations
Immunizations
Vision & Hearing Screenings/ Exams
Dental Exams

I understand that general philosophical or moral reluctance does not provide a sufficient basis for an exemption. Furthermore, I understand that exercising this right requires me to explain in writing specific religious beliefs that conflict with each state requirement. I have attached these written explanations to this waiver.

I thought in the state of illinois i do dot have to specify what religion i practice. do i have to explain my specific religious beliefs are to them? here is a copy of the exemption form that i used for wic. this is the form i prefer to use instead of their form.

(We / I) {First and Last name(s)}, as the {(parent (s) / guardian(s)} of ______________________(name of newborn child) are exercising (our/my) rights under the US Constitution, PUBLIC HEALTH LAW, ARTICLE 21. CONTROL OF ACUTE COMMUNICABLE DISEASES,TITLE VI. POLIOMYELITIS AND OTHER DISEASES, NY CLS Pub Health § 2164 (2002), to receive Religious Exemption from Vaccination, ALL injections, prophylaxis, & testing due to our genuine and sincere religious beliefs which are contrary to the practices herein required.

The US Supreme Court ruled in Sherr v. Northport-East Northport Union Free Sch. Dist., 672 F. Supp. 81, 89-90 (E.D. N.Y. 1987), that a religious belief is subject to protection even though no religious group espouses such beliefs or the fact that the religious group to which the individual professes to belong may not advocate or require such belief. Title VII of the Civil Rights Act of 1964 as amended Nov. 1, 1980; Part 1605.1-Guidelines on Discrimination Because of Religion.
