When Submitting a Conscientious exemption

Form should I fill in the vax's that my child has already had? Ds was selectively vaxed until 18mos and then I stopped all together so there are a couple series that he has completed. My state offers a Conscientious exemption that I am planing on using for enrolling him in a head start program.

Should I go ahead and fill in the vax's that he has and then sign it OR do I need to leave all the vax's blank? Does it even matter?

Personally I would leave them blank since it actually is no one's business but your own.

It is also less confusing this way and it avoids any need for explanation should someone think they have the right to question your decision.

IMO, it all depends on the way your state exemption is worded. In this case, I don't beleive it matters if you want to include since you have the choice to be selective.

Thanks Ladies.. I decided to do this and just leave it blank because it seemed that it would have been confusing to have filled some of it in. I had it notarized at dropped it off today. I guess we will see
