Exemption request = Exemption letter? Please help

I'm applying for one day at a private daycare (it's actually run in a private home) in NYC and will use the religious exemption. I think I have to use the official form from the State Education Department:

My question now is: on this form it says "Request for Religious Exemption" and "This form is for your use in applying for a religious exemption...". So it's not an exemption letter yet? Where do I have to apply? Or do I simply give this form to the daycare owner? How does this exemption request become an actual exemption letter? Thanks for your help!

Just follow the directions....you are writing in your own words why you religiously object to vaccines (thus this is the "letter" part). Then it looks like the school can either accept or deny the application. Since this is a private home daycare I don't think they have to accept it

In NY we have to apply for an exemption through the school. You submit the completed state form in place of a vaccination record, and the daycare director will either grant or deny the exemption. If granted, the school will keep the form on file, in case of a health department audit. If denied, they have to inform you (in writing) within 30 days, and explain why your request was denied; at which point you are allowed to appeal the decision. For example, if the director says your letter was too vague, you can resubmit a more detailed letter.
