I need to do a religious exemption need help

I live in mass and my son is going to need surgery soon. So I'm taking him in for a physical and they are asking for his inmunation records. I of course don't have any, so should I bring in an exemption?
Can anyone help me with writing one? I always feel intimidated by doctors so I just would like to walk in there with one.
Excuse my spelling. I was never very good at it.

You only need exemptions for school. As far as doctors are concerned, just refuse. If they ask, tell them you are religiously opposed.

You can always give them a blank vax record.
