DTaP (brand names tripedia, deptacel, infanrix): Diphtheria, Tetanus, Pertussis

MMR: Measles, Mumps and Rubella

Hib (brand names acthib, trihib, etc): infection from haemophilus influenzae type B (which can cause multiple problems such as meningitis or pneumonia, but the bug itself is called haemophilus influenzae)

Prevnar: 7 strains of pneumococcal (Streptococcus pneumoniae) bacteria (again, like hib, these pneumococcal bacteria cause different things)

Varicella (brand name Varivax): Chicken Pox

Hep B (brand names Engerix-B, Recombivax HB): Hepatitis B
Hep A (brand names Havrix, Vaqta): Hepatitis A

rotavirus (brand name rotateq): rotavirus

IPV (brand names Ipol, Poliovax): Polio

Menactra (also known as MCV4): protects against 5 strains of meningococcal bacteria- which can cause meningitis.

HPV (brand name Gardasil): Human papillomavirus

I may have forgotten some brand names but I think that covers it...
