How do I fill out new exemption form for NM?

The new form has the vaccines to mark. My oldest ds is only missing the second varicella and my youngest has everything up to his 4 yr shots he has food allergies, bad asthma and eczema. These guys were before I knew about no vax selective/delay. Do I fill out the form to what each is missing or fill them out so they match?

There is no reasons needed/stated on the form, it's just "I object to the following". I just checked that I was opposed to everything, and am hoping for the best. DD had everything except the boosters. I too worried a bit about everything matching but then, I also didn't want to open myself up to problems with younger dd who up until now has only had dtap.

I've been told there are no problems with this.

Funnie I wanted to update you we got our approval in the mail today. Super easy and it worked out just fine.
