who do I file an exemption with?

My DD will be returning to preschool next month. She was "up to date" on all vaccines at age 2, before I knew better. Now that she will be turning 4 next month, she is "due" for more... I will not have her vaccinated again.

We are in KY. Who the heck do I file paperwork with to get the exemption? I've seen the paperwork online before, but who do I turn it in to? Is it a doctor thing? A state thing? A preschool thing? I'm so confused.

We gave our exemption form to the daycare director, along with the other forms required at the beginning of the year. No doctor's signature was required on it, and I used a modified form that is provided by our state. There was a medical form with places for vax, signed by the doctor, but that was not the exemption form. Where the vaxes go on the medical form go, the doc just put a line through it and wrote, "parents deny" or something like that.

SO, 2 forms: 1 exemption (created and signed by me), and 1 medical form, signed by doc, with line though vax section. And about a dozen other forms, but those were not related to vaxes. Such is state-run daycare.

You get it from the health dept. and then give it to the school(s) she is attending. At least that's how it works in my state.
