School system requires visit w/nurse for religious exemption, question...

This is Fort Mill, SC school district.
According to the school district I'm getting ready to enroll my children in, I have to be "interviewed" by a nurse at the local health department to get the religious exemption form to give to the school. My question is: do they really have the right to ASK me anything? Should I really answer any questions they ask me? Should I ask for the questions in writing and respond in writing?
My visit is tomorrow, so any quick help would be greatly appreciated!

What state are you in? Add it to the title line, that'll help get knowledgeable folks to answer. Or search this forum for the name of your state and see what other folks' experiences have been (Search is near the upper right hand corner).

Can you search for legal aid? At the very least I don't think you should answer anything. But I do not know your state and am not a lawyer.

In your shoes I would hire a lawyer or spend some time putting together a very strong legal document stating you will not be interviewed as these are your religious beliefs. I would also complain all the way up the line in writing. If for some reason it came to be that I was in the room w/ the nurse I would say over and over again, "I will not answer b/c these are my religious beliefs. You cannot question my religious beliefs. Please give me the form now."

Answer:
A Religious Exemption form (DHEC form 1126) can be obtained by requesting a form in person from any county public health department. Contact numbers for countypublic health departments can be found at http://www.scdhec.gov/health/countymap.htm.
Forms cannot be e-mailed or mailed. Follow the instructions on the form. Don't forget to have the document notarized.

Yeah, I had to actually make an appointment, which I thought was a little bizarre. I just don't want the nurse to "trap" me with questioning me or anything. I read somewhere else on these forums that I should ask for any questions to be put in writing and not to answer anything verbally.
I'm just hoping it's a cut and dry thing and they'll just hand me the form.

I'll definitely post back tomorrow after my visit.

you're welcome... I hate beaurocratic bullies. Most threads said the same thing, they warned you your kid could die of chicken pox, told you you'd be booted during an outbreak (first to leave, last back in) and that your kid would fail because it's unexcused. All crap!

They cannot withhold an exemption; it is not supposed to be an interview, but a lecture "you are a crappy parent; your child will DIE, DIE, DIE if they are not vaccinated."

If they tried to question me, I'd probably say, "I'm here for a Form 1126 and I will not answer any personal questions" and, if necessary "you cannot legally withhold the Form 1126 if I do not answer your questions."
