4 year old's HUGE cavities - how will they fix these???? (sorry - long)

I am soooo sick to my stomach right now realizing this is totally my fault and I can't believe I let them get this bad.
DS has been through a lot. It took 3 years and what seemed like a million appointments with specialists to finally get my son's diagnosis for extreme failure to thrive - he has a brainstem tumour. They found it one year ago and he's been going through weekly chemotherapy since. Prior to his diagnosis he had a chronic vomitting problem due to a sensitive gag reflex. On top of the vomitting he hated having his teeth brushed and it was a battle I wasn't up for fighting daily with everything else we were going through. So, his teeth didn't get brushed as often as they should have been which is my fault. And since he and we spent so much time in hospitals and doctor's offices I couldn't bare to put him through more so didn't take him to the dentist for check-ups like I should have been. So, there are my excuses to make myself feel justified...even though I still feel terrible.

So, his teeth:
1. he has really bad enamel breakdown at the gum line of the two teeth adjacent to the two front teeth. they have a yellowish substance in them which looks nasty and I am not capable of removing. Not sure if there are cavities there or not.
2. And 2 very noticeable cavities on his back 2 lower molars on the left. The last molar has such a big hole that half the tooth is gone right down to the gum. you can't see this when he opens his mouth because his tongue blocks it which I guess is why I've never noticed it. And the adjacent molar at the front back side is gone as well.

UGH! I can't believe this has happened. He's been through so much. I tried to call our dentist to book him an appointment but they are on holidays for the next 2 weeks. I am sure he will have to be knocked out to fix these right? Do you think they will just pull the one molar out since it's so bad? He is a big risk to knock out because of his tumour which causes sleep apnea. Last time he had an MRI he had major problems - kept turning blue and his oxygen levels kept dropping really low from the anaesthetic. He can't have sedation (becasue of the sleep apnea) or gas (because it causes emergence delerium in him). I am guessing he will have to go to the children's hospital to get them fixed?

They don't bother him at all. I am not sure how the tooth can be rotted away so bad and not cause him pain. I am so worried for him though. I'm not sure how I could have done better with a child who for months vomitted several times a day, but I shouldn't have let this happen.

Please don't beat yourself up about this! You seem like a wonderful, caring, sensitive mother and your little boy is blessed to have you. You and your son have been through SO much. I can't even imagine how gut-wrenching all of this must have been for you.

My youngest son (will be 2 on Sunday) had his first dental visit a couple of weeks ago. He has a couple of yellowish spots on his middle front teeth and they told me it is an enamel defect that he was born with (likely a result of antibiotics I took while pregnant with him) but there is no decay. It could be the same for your son. As for the cavities that you can see, I don't know what they will suggest as it seems like each dentist does things differently. I hope your dentist is supportive and is sensitive to all that your child has been through. Please keep us posted.
