What to write in a letter of intent?

We're in Hawaii and I know I need to submit one (in fact public school starts at the end of July and we're at July 27 right now, so I hope I'm not late? It might even be starting this week, I'm not sure.)

Can anyone tell me what goes into a letter of intent? And do I just take it to the local school to me? Thanks for any advice!
