Active duty military - I don't want to be the guinea pig

Please help. I am active duty Air Force, and I do not want to get tested with the swine flu vaccine in a couple of months. The only way I know prevent vaccination is to file a religious waiver. I do hold religious beliefs that are contrary to the practice of vaccination, but I have never filed for a waiver. I have been on active duty for a little over a year. Before last year's mandatory vaccinations, I didn't know the religious waiver option existed. My religious beliefs evolved before I came onto active duty, but after I signed paperwork binding me to the military, 8 years before. Long story - I was reserve for a long time.

I know that the link here http://www.vaccines.mil/documents/969r40_562.pdf outlines the active duty vaccination waiver requirements, but it doesn't really go into HOW to file. Who do I talk to? Who do I submit my letter to? How do I get the process going? Has anyone ever known anyone who has successfully gotten a religious waiver approved?

I am willing to separate from the military over this, if necessary. I just don't want to go to jail for disobeying an order. Please help!

I'm fairly new to this, especially being a dependent. Have you tried contacting a chaplain or your current spiritual leader?
