Babies with reflux like to nurse frequently in small amounts. The problem with this is that they can have issues gaining because they tend to get a lot of foremilk rather than the more fatty hindmilk.
My son has a heart defect, which requires more than normal calories and he also had reflux. We got reflux meds (Zantac tends to be cheaper than Prevacid) and I also pumped foremilk before I nursed him. This worked well for us and he started gaining. Another option is you can do breast compressions while nursing to get him more hindmilk.
The formula can make you produce less milk, you might want to pump extra if you are giving him formula.
Reflux meds really helped my baby. I could tell a huge difference in how comfortable and content he seemed.
I would ask my Ped about the rash, what kind of laundry detergent are you using?
