AAP Vax Form-revised version

Hi all!
We actually found a GREAT family doc who is totally okay with us not vaxing. hooray! Went in today for DD's preschool physical and "well child" visit. Since it's a family practice they didn't even have refusal to vax forms! She had no idea what to do with me, but knew I needed to sign something for the practice. I told her I had a form we used at our former dr.'s office. It was the AAP regular vac form but with an additional paragraph included. Something like I also understand the potential harm FROM vax ??? I got the form from this forum, so I'm hoping someone has some idea what I'm talking about. I can't find it anywhere in my files.

I don't understand. If they don't have a standard form, then why would you need to sign one? My DC goes to a FP Doc also, and we've never signed anything. I understand offering up the revised version at an office that has given you an unacceptable form to sign. However, I would never volunteer to sign something. Is signing a waiver a condition of them keeping you as patients?

dont' sign it .. just casually forget it .. dont' adress it unless they push the idea and be very careful about what you are signing, if it is not understandable and there is legal jargon i would seek out a lawyer... there is no reason someone should have to sign a waiver.. i thank mother earth everyday for the doctor i have...

no, she didn't say they would drop me. I guess I want to be pro-active about this. She is really my ONLY option out here and it took forever to find her. I didn't want her digging around her office and talking to the other doctors in that practice that are totally NOT non-vax friendly. She was totally cool with our choices and not all that concern about the TYPE of form, she just knew I needed to sign something. I'm not willing to fight the fight on this, and thought I'd appear helpful. In turn, I can do what I'm comfortable with and submit something to my liking. We don't do any other visits unless totally needed. So this is basically a yearly physical for school.

I can also "forget" to bring it in (I have appt. #2 with DS5 tomorrow for the same thing.) I just want to be prepared. Helps me feel more confident.

So back to my original question: does anyone know what form I'm talking about and where I can find it?

You can use the standard form and cross out the offensive line (the AAP says their form can be modified, our doc who uses the AAP form routinely was fine with that), that's the easiest way. Another easy way is to say Dr. X offered vaccinations for A, B, C, and D in accordance with the CDC recommended schedule and I, so-and-so's parent, declined. Covers them (they offered), but nothing offensive.

On a side note: my doc and I totally misunderstood each other. She brought it up again today at my son's appt. Apparently She thought we needed the form for school. I told her, I thought she wanted for the practice. (we have to get a notarized VA form for school) So it was fine but she decided to put the letter (revised one in the link above) in the file in case one of the other doctors had to see us and was questing the vax situation. That way they would know my doc. was aware and had already addressed it.
