Practice, practice. Get good and quick at it by practicing on a teddy bear or doll, then make sure your non-pinning hand is under the dipe (so if you go too far, you pin your hand and not baby).
It seems daunting, but really is easy. I never feel comfy leaving baby coverless with a snappy once they're moving, so it is nice to have pins around. (also a good party trick-amaze your friends! lol they already think you're a freak for using cloth, why not really blow their minds with an origami-folded flat?!

I exclusively used pins with flats and prefolds when I was CDing my daughters (except for a half-dozen fitteds that I sewed myself later on- but those were mainly used by DH. 6 diapers don't go very far with 2 babies!)
I had the changing table set up so that DD could stand up and hold onto the dresser next to the changing table while I took off the old diaper and put on the new one. That kept them a little less wiggly.
By the time DD1 was a toddler (and certainly by the time DD2 was a toddler!) I was an expert diaper pinner. I could pin quickly and efficiently. I never once stabbed either baby, although I did stab myself on the finger a few times. I always put my finger between the diaper and the baby while pinning.
IMO, pins are the BEST way to fasten diapers with toddlers who liked to undress themselves and deposit diaper contents in various rooms of the house (or undress and then pee on the floor.) Velcro opens up easily. Snaps can be unfastened with enough brute force by a determined toddler. Pins are toddler-proof! (I also liked the disposable diapers with tape that didn't unfasten easily, but none of the modern diapers have that anymore.)
