Help, I thought I was doing the right thing, but . . .

Sorry this is long.

DS2, 9 mos old, had a high fever for 2 days. Like spiking to 106, and hovering around 102. It was better this morning, then started going up again, so I took him to Dr. Our regular (awesome) ped was not in the office, so we saw another Dr.

No other symptoms, so she suggests possibility of UTI. I kind of think she went for this just because DS is intact. But he really has no other symptoms except that he doesn't want any solid foods. She then says she hates the idea of having to catheterize him, so she wants to bag him for a urine sample instead. If the sample is negative, great. If not, then she'd consider the need to do a catheter for a clean sample. I agreed, thinking this would be non-invasive.

The long and short of it is that he was retracted by the nurse who was cleaning him with betadine to get a clean sample. I kind of freaked, and she said she was trying to make sure he was really clean so that the sample wouldn't yield a false positive and we'd avoid a more painful situation with a catheter.

I feel so terrible for letting this happen. DS seems okay now, but we never even got the sample because he just refused to pee in the bag. The nurse sent me home with some supplies to try to get a sample myself at home, and ds still has a fever.

What should I be doing at this point? I don't want to completely discount the possibility of UTI, since he's had a high fever, but I also feel like this Dr. jumped to that conclusion just based on his intact status. I'm afraid that if I bring a sample to the lab that is positive for bacteria they'll recommend a cath and that will be worse.

Anyone have any advice? I'm in CA and supposedly only 50% of boys here are circed, so I took for granted that I wouldn't run into something like this.

I always think, "What would they do if he were a girl?" And, in a case like this, I think they would look at the possibility of a UTI. I don't think they are necessarily being overcautious about that particuarly. I can't believe he was retracted for a bag! Jeez, that's supposed to be "safe".

I'd do the baggy still, but obviously don't retract. The foreskin should heal up in a few days. Sometime I get the feeling they just look for excuses to retract.

Actually in California only about 30% of boys are circed.

So he is eating/drinking and peeing okay, right? I highly doubt just a fever would indicate a UTI but there's no harm in getting a bagged urine sample for home.
If you're breastfeeding, continue offering breastmilk or any fluids he may want. I really think if he had a uti his urine would look/smell highly concentrated or he would grab at his diaper due to pain, especially since he's 9 months old. Regardless of if you treat the fever w/meds or just treat with comfort measures, continue monitoring his temp and fluids.

Again, I really don't think it's a uti. Some random virus seems more likely (stomach or resp.) - which would cause a fever too. I know of two people out of state right now who have a "summer cold" which led to an intestinal virus. Maybe your ds has had exposure to that. Some babies spike a temp when cutting teeth or ear infections, etc. Why do they always assume uti???

Hope he's feeling better soon and I hope you'll bring this incident up with your regular doctor.

Beru, I think you're right. When I had a chance to process everything and think about it rationally, I did see where the doc was coming from. I think UTI was not an unreasonable thing to rule out.

Night_Nurse, we got the results back from his urine sample (I did the bag thing at home this morning--WITHOUT retracting and he was negative for a UTI. Hooray!

So he seems to be a bit perkier today, and the fever has not spiked up high again. So I think it must be some other little virus that no one else here was affected by.

I'm trying to look on the bright side, because I didn't see any bleeding or obvious injury to his penis, and I know the nurse was truly trying to help me avoid what she thought would be a more painful and invasive procedure. It's just awful to see your little one get hurt and have to second-guess yourself.
