I think we signed the AAP refusal form...

I was reading some threads here and came across a couple on it. At out last visit to the new pedi (who is very holistic and anti-vax) he asked us our plans for vaccinating. We said we were still thinking about it, but we'd probably do delayed/selective vaxing. He said that's fine and we could change our minds later, but could you sign this form saying that you're not vaxing please so that I don't get in trouble. I was nursing the baby so he gave it to DH. DH read it and signed, and I signed without reading, figuring if there was a problem DH would have noticed when he was reading.

Now I don't know for sure what the form said, but after reading the other threads and worries about CPS, I'm a bit worried about the form. I don't expect CPS to have any reason to deal with us in the first place, but if something ever did happen, I don't think I want something saying I knowingly put my child at risk (if that is what it said) in his files. Has having it in there every actually been a problem for anyone?

Update: I asked DH about it when he got home. I had him read the AAP one (the form we signed looked different) and he said it wasn't the same and that he was pretty sure there was nothing in there about endangering our child.

I'm pretty sure I've signed one before. I, too, was nursing my LO and didn't read it before signing. We don't take the kids to that ped anymore so I'm not too worried about it. But it bothers me to know that I signed something like that and it's floating around out there...

i signed it but i always change it. where it says that i may be endangering my child i always add "or may not be." and i initial the changes just to screw with them!
