It appears that all you need is a letter stating that you are religiously opposed. Don't write about anything specific; you are simply religiously opposed to the practice of vaccination. I didn't see that you'll need it notarized, but you may need to read through the law better (I just skimmed for you.)

If I were you, I would just keep it onhand and turn it in when you are asked for proof of immunization. Then turn it in (after making a copy.)

If this is a private pre school they dont have to accept the exemtion. If it is public then there should not be a problem.

Hey, you shouldn't have a problem (thanks to all of those that have petitioned for us!). I always just give a recent physical, a copy of the state law and my religious waiver........have NEVER had a problem, good luck

Actually, I don't think that's true in Virginia. Someone correct me if I'm wrong here, but the Virginia law states:

"School" means

(i) any public school from kindergarten through grade 12 operated under the authority of any locality within the Commonwealth,
(ii) any private or religious school that offers instruction at any level or grade from kindergarten through grade twelve, and
(iii) any private or religious nursery school or preschool, or any private or religious child-care center required to be licensed by the Commonwealth.

I would add that you probably need to use the form linked in the NVIC page (it's the last link) or from the VA health page - same form: http://www.vdh.virginia.gov/epidemio...quirements.htm (5th link in the "quick links" on the right) . It says generic "affidavit" on the NVIC page in that first paragraph, but it's the form you need (although, if someone has submitted a letter instead please post). The school entrance/physical form also has a little box stating religious exemption is acceptable (well, something to that effect) by using that form. And it needs to be notorized.

I've had no problems with daycare or preschool, handing the notorized form in along with other paperwork when that is due - as another poster noted, "school" is defined as private preschool, or any private daycare required to be licensed in VA.
