green card question

so i requested ds's green card (immunuzation record) for school. doc said it'd be ready in a few days, so i called to have them mail it. it showed up, but blank. doc signed and checked religous exemption, but the vax area is blank.

thing is, ds was fully vaced thru his 6 month appointment, then none after. he recieved all those vaccines at a different doc, switching when we needed a no-vax friendly one.

so i guess our current doctor doesn't have ds;s records from his previous doc and assumed we've always been no-vax (although i know i told him at our first appt there that ds had recieved some). i imagine that's long been forgotten though.

so. i see this blank green card as good as far as us claiming religous exemption, less red flags than partially vaxed, right?

do the docs usually cross out the blanks or anything? is he possibly expecting me to fill it in? is that what you're supposed to do?

I would simply submit a religious exemption in it's place. You do not need to bring up previous vaccines.
