Need calorie dense smoothie/food for gfcf child

My 5 year old is extremely underweight, and his asthma meds seem to suppress his appetite. While we are busy trying everything under the sun to eliminate the asthma meds, I'm looking for high calorie smoothies or snacks to help him get enough in. His diet is so restrictive. He also doesn't do well with sugar, so he's basically off dairy, gluten & sugar. We've even considered putting him on hypoallergenic formula but the ingredient list puts me off. Anyone have suggestions?

No recipe but...
A green smoothie with coconut milk and avocado.
Cream soups made with coconut milk.
All I can think of atm is coconut milk. Maybe nuts as well for breading on items?

Does he do meats? We do a lot of sausage, bacon, etc. for fatty foods. Also pine nuts (we make "ricotta" out of it -- check in the allergy recipes thread on the Resources sticky) and use it on pizza (we use www.buckwheatpete.com pita recipe as the crust). Our smoothies are coconut milk, strawberries, blueberries, banana and if the kids aren't looking, I add spinach and a capsule of probiotics). I also make coconut milk yogurt and that goes in there sometimes. My DD2 likes to dip fruit in the yogurt as well. Peanut butter-coconut balls sweetened with honey are a good treat (not sure if you're okay with honey as a sweetener). I have recipes on my blog: www.kathysrecipebox.com, searchable by allergen.

High-calorie smoothie/shake idea:

1 very ripe banana
1/2 cup or more unsweetened soy milk or unsweetened So Delicious coconut milk beverage
1 tbs. ground flax seed
1 scoop Purely Decadent dairy-free coconut milk ice cream (sweetened with agave) -OR-
1 scoop It's Soy Delicious dairy-free vanilla ice cream (sweetened with fruit juice, rice and tapioca syrup)

Feel free to add nutrient-dense ingredients like peanut butter, medjool dates, berries (fresh or frozen), and unsweetened soy protein powder.

Did you find a workable recipe? I can fit about 450 calories into less than 6 ounces. Here's what I use:

soy yogurt
maple syrup
coconut oil
protein powder
brewer's yeast

Yum!

Hmmm... asthma, issues with dairy and gluten, digestion issues (??) with sugar, underweight - have you considered supping zinc? That mix sounds like low stomach acid, perhaps. Addressing DS' zinc deficiency got us back several foods (better sugar tolerance - not important for a food, but a good sign of better digestion), and he's clearly getting more nutrients from his food now.

ETA: Sorry, the reason I wonder about stomach acid is that many asthma meds are anti-histamine, and histamine triggers stomach acid. You might also try digestive enzymes or kefir/fermented foods to help with digestion. That way the calories you get into him would get better absorbed and used.

Our smoothies usually include:

coconut milk
homemade almond/cashew milk
coconut kefir(store bought for now)
1/2 bananna
bluberries or
strawbwrries
spinach or kale
flax oil in moderation
coconut oil
chia seeds
