Filling in your own vaccination record?

We just got back from DD's 18 month well baby visit, and I had misplaced her card since we just moved. They gave me a blank one with the printout from the state records, and told me I could fill it out myself.

Really.

Is this normal? I could just fill in that she's had everything. WTF.

I've had a blank sheet given to me when I was an adult (that I filled in myself - I think for college), so I guess it happens.

I think they aren't thinking anyone will forge anything... that everyone wants to be up to date on vaccines and will just fill in the correct info I guess

It's just funny ... the whole thing started because I asked for a correct record. The state's printout didn't look right to me - the nurse said "that's why we ask that you bring your written record every time, it's the most accurate."

I'm not going to forge anything, but it just struck me as odd. I'm actually going to hold onto the blank one and try to find the old one. It might come in handy in the future.

some of the records on ours say "MMG" hand-written and the date, also hand-written.

i mean, there's no way to trace it and in fact, i know they marked him off for one before they determined they were out so he has an extra hib on there he didn't get.

Now they have the national registry, if you put false information on the form, they'd know right away when they checked the database right? or are some states not using this yet?

I don't think all states have one and many have been started in the last 10 years so older children will not be in there and in most states you can opt out if you want.

I have often thought about forging one of those. Fortunately, the exemption process in my state is pretty easy but if I were to move to MS or WV I'd do it in a .
