The design you're talking about is of the University of Chicago Hospital, which has a very high circumcision rate. This may have something to do with the daily protest by intactivist Dan Strandjord at the hospital.

They didn't cut the whole head off; they ripped the skin off and left the bleeding flesh exposed to the elements.

Well I would say it is a con-circ shirt with a sarcastic/ironic tone to it. My interpretation is that it is trying to show exactly what circ is like: - ripping the skin off a proud and healthy creature - so to speak.. I am guessing they are trying to make a graphic illustration of the absurdity of it all..
There is no way I could interpret that as a pro-circ shirt. Also it is listed under "intactivist" items.
