I had been given the run-around by phone from the Health Department, after 5 phone calls and no returns, they accidentally canceled my appt this morning. But decided to let me in since I made the appt yesterday
The nurse reads the vax info sheet, emphasizing that DD's next polio and MMR was the last booster in the series. She also stressed how serious whooping cough is and there is a local outbreak, so they are trying to get 100% immunization... hmmmm... nod & smile.
Then I wrote this statement:
"I, ______, mother of _________, have been provided with the Vaccine Information Statement for DTAP, polio, chicken pox, and MMR vaccines. I understand the risks and benifits of having these vaccines and choose to decline based on religious grounds. Sign, Date"
Then she stamped my exemption sheet and wrote in DD's info.
Ta. Da.
