Ohio Exemption for school HELP

My kids are enrolled in Boss a online school. I still have to fill out a exemption form and I recieved it in the mail today and they are asking for a Physician signature and I didnt think I had to do that is also says

I the parent of the below named child hereby ovject to the immunizations listed for the following reason

Please list the immunization to be exempter and the reason WHY.

It does state part 3 and 4 of the law I signed it as is should I send it back or is there something that needs to be written in there for reason?

in ohio you have all 3 exepmtions available to you.

according to this, it's either or, so you don't need a phys sig unless you are using medical exemption.

Finally!!! I found the state form! Page 9. The very bottom line.

I would say something like, thank you for trying to ensure that your paperwork is in order. Here is the official form for the state, so it should suffice.
