I need IMMEDIATE help!!!!

My friend is over here helping me clean and she mentioned in passing that her 17 month old baby is getting surgery next week. She stated that the pedi and urologist stated that his penis looked like it hadn't been circed and needed to be re-done again. I have talked to her for the past 30 minutes about this and I hope that I have at least given her some food for thought because she is concerned now.

Her DS has obviously had a very loose circumcision so currently there is still foreskin that covers the head of his penis completely. Well everytime this LO has been to his pedi he has been retracted which I'm sure you know what this has caused. The foreskin is currently swollen and red and the pedi is saying that this is DIAPER RASH!!!:. So not only has he been forcefully retracted- they have been told to put diaper rash cream which is getting inside of his penis.
He screams when his diaper is changed and the pedi has told the mom to retract him to clean it.
Surgery is scheduled for the 18th but now that I have told her all I know- I think she is going to cancel it or at least put it off for right now.

I need IMMEDIATE advice while she is here so she can read this....

1. Why foreskin is important
2. How she needs to start caring for it. (SHe is going to put him in the bath tonight to get as much of the cream out of there).
3. What to do for the current redness and swelling. (Could it be a fungal infection because of the diaper rash cream?) Would using nystatin on it help at this point? Or just fresh air???

1) So many reasons why it is important I cant think of them all maybe someone else will come along with the list.

2) It should be left alone and not retracted just swished in the bath. Avoid using soap or sitting in soapy water.

3) He could have yeast yes and nystatin will help if it is. OTC monistat 7 day can also be helpful with yeast.

If more is taken off there is a huge risk he will end up with to tight erections. He is very lucky to have a lot of foreskin left so that he has a better chance of that not happening and it will make it easier should he decide to restore.

what should she do immediately to heal his penis?
She can use the same tips found here for an intact penis to help him heal http://www.mothering.com/discussions...d.php?t=764732

If he has enough foreskin left that it covers the glans it should be treated as an intact penis and never retracted and never let do it either.

I cannot find any links for this type of thing right now. But what is important is getting him pain free as soon as possible and not cutting any more of his skin off because that can lead to nothing but problems for him later in his sex life.

Here is a site where there is contact information for dr's who will help her either through email or phone call free of charge. They know exactly what they are talking about. http://www.doctorsopposingcircumcisi...t/contact.html
I dont have Marilyn Milos information or I would include that as well. She is a nurse and founder of no circ. Had her boys done before finding out it wasnt needed.

Does she understand the anatomy of the infant penis?

It's important to know the anatomical parts and their function to have a generalized understanding of the foreskin.
Have her watch this: http://www.doctorsopposingcircumcisi...o/prepuce.html
It's a 19 minute video on the anatomy and function of the foreskin.

Once she understands the function and anatomy of the foreskin, she'll understand what has happened in the circumcision.

The foreskin is an immature organ at birth. It's only function is to allow the exit of urine. Regraction of the foreskin and exposure of the glans (head) of the penis is not needed until much later for sexual activity. (Some men maintain a non-retractive foreskin into adulthood and this is not problematic for them. For a man with this situation who finds it problematic, there are steriod creams and gentle stretching exercises to cause the foreskin opening to stretch and allow the skin to retract.)

Since the foreskin is adhered to the penis, there is no need to clean, as there is no space to gather any foreign material.

Upon circumcision, the foreskin is seperated from the penis by a probe and the foreskin is cut off. Remaining foreskin, in the body's infinite wisdom, attempts to heal from the trauma of the circumcision and re-adheres to the shaft/glans of the penis. These adhesions should NOT be torn apart. Doing so physically tears the tender mucousal tissue and opens up an avenue of infection. Repeated tearing of this tissue can cause the formation of scar tissue - which is not elastic - and can cause the foreskin to not be able to release or retract. The only person who should ever slide the foreskin (or remnant of foreskin) back to visualize the glans is the person who owns the penis/foreskin. They are the only ones able to know what is painful and what is not.

There is a huge amount of ignorance about normal genital development of boys in the US. The idea that circumcision is beneficial is evidence of this. The idea that any amount of foreskin needs to be pulled back and "cleaned" is also evidence of this.

We don't spread the inner labia of girls and vigorously "clean" them. We don't spread apart their tender genital tissue with such force to cause bleeding and inflammation during a well baby check or recommend this as home care by parents. I do not understand why doctors don't recognize that "care" should not cause pain, inflammation, bleeding and infection.

Here is a link about the development of retraction: http://www.doctorsopposingcircumcisi...etraction.html

He is lucky to have his foreskin. If they take off more he is at risk of having tight/painful erections is an adult.

Find a doctor who knows something about the proper care of foreskins. Her current doctor is not only clueless, but dangerous. He/she is obviously not keeping up with the current standards of medical care.

If she can be forceful and control the doctor patient relationship with her current doctor, she can try limiting his care to treating the redness and discomfort only, while she finds another doctor. This might include testing to see what is causing the redness, then treating appropriately and topical ointments to relieve pain temporarily, but I am no doctor nor do I have knowledge in this area.

Otherwise, go to another doctor.

Best wishes to her. She should never have been put in this position.

Regards

The mom should Google "circumcision damage" and see that painfully tight circumcision is a horror.

Treat the problem medically, not surgically, as one would with any other body part on a male or any part of a female.

First-- bless his heart.

She should be on the phone/internert looking for a new pedi.
As for immediate care- she could take her son to the doctor and tell him that she is very unhappy with the situation and tell him that she does not want another circumcision. Get him to swab the TIP of his penis. Do not allow him to retract his forskin. She could even take the swab herself. Can you go WITH her to help her at the doctor? It would probally give her more confidence if you were there to back her up and make her a bit more comfortable.

Does your area have a pediatric urologist? That might be a great place to go for immediate help.
