sick cat - at a loss - PLEASE HELP

Please help us with ideas even vet is stumped!!

Here is what we know:

Sami was a pregnant stray when we adopted her 9 years ago, at roughly age 2 so she is roughly age 11. She was neg for FeLIV and feline leuekemia (sp?). Everything was going normally except she was a snuffler. Our vet said it was mild allergies and not a big deal.

A year ago she started mild weight loss, which I thought was from us moving from a little trailer to a house with steps. I lost 5 pounds from the steps, and she follows me around so I thought her slimming was the same thing.

6 weeks ago she starts a dramatic weight decline. Roughly 2 weeks later she has signs of an upper respiratory infection. Went to vet, treated with antibiotics and IV fluids at home. 2 days after finishing meds she's sick again and peeing blood. Start meds and fluids again. 2 days after finishing meds she is horrible again, so they try a shot antibiotic, which doesn't help. Go back for previous antibiotic, and give that and IV fluids. Her appetite went to crap every time she was off meds, but after 7 days of little eating she started eating again Monday night. For a while all she'd eat was wet food, but now she eats kitten food. She is barely six pounds and skin and bones. A side note is that she no longer snuffles. However, what does she have? Any guesses? Her blood work was normal, and any further test, according to the vet ( a different vet from the allergy diagnosis) is a shot in the dark. I want to help her but I can barely feed my kids right now dh has lost 2 jobs in the past year so I want to guess wisely so we don't waste money. We have other cats and none of them have caught this, so it seems as though her immune system is impaired or something. She seems to do okay on fluids and antibiotics. A few days ago she was having labored breathing but the vet checked her and said her heart and lungs sounded normal.

She is my best friend. I need to help her.

What can it be?

Peeing blood is worrisome...severe UTI, bladder/kidney stones, possible renal failure, Addisons disease? What is her white cell count? I can't imagine her blood work is normal if she is bleeding and needs constant antibotics. Has she had a CBC or the gen. panel of blood drawn? Perhaps get another opinion? Has she had any xrays to rule out masses?

What blood work did they do?
Any changes in hair coat?
Any neurologic changes / changes in her gait?
Any vomiting? Diarrhea? Blood in stool?
Does the urine have any strange smell? (sweet smell/musty)
Did the DVM do a urinalysis?
Are you giving fluids at home?
How is her heart rhythm? Pulses?
Any swollen lymph nodes?
Any decreased breath sounds?
Healthy mouth?
Any new chemicals in the house / on the lawn / in the garage?
Does she go outside?
Is she hiding? Sleeping a lot?
Does she seem tender when you touch her abdomen?

If you can get her to eat it, Hills A/D is a good critical care food that is reasonably priced, good place to start. Anything that will encourage her to drink is a good thing, this might mean letting her drink from the faucet or changing her water several times per day.

How has she been since Monday when she started eating again? Is she acting normal again? Is she symptomatic?

I don't know how her blood work could be normal. If she had an infection her white count would be off. Have you had any of the geriatric cat blood panels done on her?

I was wrong about the peeing blood - that is one of the other cats as I discovered this morning. So he is going to the vet today.

But about Sami:

She didn't have a CBC because the vet said it would be inconclusive since she had an infection and that would change her white somehting or other. She had her levels done, liver, kidneys, glucose, etc. and all were normal. She did not have at x-ray because he said she is so thin he thinks he could feel a mass (and he felt he all over to check) if one was there but we havn't ruled out getting an x-ray if she doesn't improve.

Only from lack of grroming. When she is better on the meds and grrooms her coat is normal. However, she is losing small patches of hair in spots.

Her one back leg is getting bad arthritis and she can't really bend it, but she bares full wait on it.

No vomiting or diarrhea or bloody stool

Can't tell about the urine smell. Although she's used the box in front of me and it seemed normal.

No urinalysis

Yes to fluids at home via IV

Her heart and such is great

No swollen lymph nodes

Normal breathing, this is actually better since the snuffling stopped

Her mouth is nasty because she goes so long without drinking I have been letting her drink out of the tub when she will and squirting water in her mouth and wiping her teeth but it still smells

No new chemicals - I am sensitive I clean with vinegar and baking soda

She is only indoors

She is hiding and sleeping when her fevers up and she's bad. When she is doing better on the meds she follows me around and bosses me like normal, with lots of chirping.

The only place that's tender is the arthritic leg.

The first bad round she ate the A/D, the other times she wouldn't touch it.

She does like to drink out of the tub faucet when she feels well.

She is acting fairly normal since she started eating but she is still losing wait and her arthritis is worse. But she is chirping and following me around again, as well as coming when I call her. I don't know what a geriatric cat panel is - is that the kidney/liver/etc. test they did or as that another one?

Thanks for everyone's interest. I think my concern is mainly that the day after going off meds her fever and other symptoms come roaring back and each time it takes her longer to start eating again. And she is still losing wait.

Oh she had frontline for the first time 5 days before all this started - she has had advantage before but not frontline.

I would next probably go to a lateral chest, lateral abd & VD abd xrays.

The blood chemistry was perfect? Has he talked to you about possible adrenal issues?

Frontline issues present differently, so it doesn't sound like that is the issue.

Are you actually giving fluids into a vein via IV at home, or do you just give them under her skin (which is not IV)? If you're giving under her skin, how much and how long does it take for the lump to go away.

How is she tolerating her antibiotics?
